Kounis syndrome and systemic mastocytosis: one step from elective lipoma resection to cardiac arrest

چکیده
Background Kounis Syndrome manifests as unstable vasospastic or nonvasospastic angina and is caused by the release of inflammatory mediators following an allergic reaction. A variety of causes have been reported including antibiotics, general anesthetics, latex, insect stings, drug eluting stents, etc. To the best of our knowledge, Kounis syndrome in the setting of systemic mastocytosis has not been previously published. We describe the case of a 52yearold male, who developed Kounis syndrome perioperatively and was subsequently diagnosed with systemic mastocytosis.

Lepirudin anaphylaxis and Kounis syndrome.
To the Editor: In an ongoing lepirudin pharmacosurveillance program, Greinacher et al1 reported that 4 patients experienced fatal anaphylaxis after reexposure to lepirudin and died shortly after the onset of the event. Three patients died from acute cardiorespiratory arrest and 1 from acute myocardial infarction. These cases are characteristic examples of drug-induced Kounis syndrome2—the concu...
متن کاملAnesthesia Management in a Patient Diagnosed with Kounis-Zavras Syndrome and Who has Brittle Asthma and Samter Triad
In Kounis-Zavras syndrome, vasospastic mediators released from mast cells as response to allergic stimulus lead to coronary vasospasm and result in acute coronary syndrome or angina pectoris [1]. In this case report, anesthesia management in a 35 year old female patient with Kounis-Zavras syndrome who had Samter triad and multiple drug allergy and had arrest previously due to drug allergy and h...
متن کاملKounis syndrome: A primary cause for the anaphylactic shock.
In the important paper of Kuda et al. [1] published in ”Cardiology Journal” it was found that vasospasm-induced coronary flow reduction is a major contributory factor in anaphylactic ventricular dysfunction. The authors concluded that during anaphylaxis the ensuing left ventricular dysfunction is attributed mainly to coronary vasoconstriction-induced myocardial ischemia. These and other experim...
متن کاملSudden death in a patient with amyloidosis of the cardiac conduction system.
A 54 year old man had generalised systemic amyloidosis secondary to bilateral basal bronchiectasis of the lungs. He died after an unexpected asystolic cardiac arrest. Necropsy showed extensive amyloid deposition in the cardiac conduction system.
متن کاملCardiac arrest caused by nafamostat mesilate: Kounis syndrome in the dialysis room?
Nafamostat is a broad spectrum serine protease inhibitor with anticoagulant properties that is used during hemodialysis to prevent the proteolysis of fibrinogen into fibrin without increasing the risk of major bleeding, in patients prone to bleeding. In the very interesting report published in this Journal [1] concerning a 65-yearold man with diabetic neuropathy who was subjected to buttock abs...
